Support for the first days home

We're non-medical, so we work alongside the discharge plan and any home health agency involved.

What we can help with

  • Sitting with a patient during a hospital or rehab stay, where the facility allows
  • A ride home at discharge
  • Groceries and prescription pickup on the way home
  • Meals ready and the home tidy
  • Companionship through the first days home, including overnight
  • Rides to follow-up appointments
  • Verbal medication reminders
  • Updates for the family

What we don't do

  • Bathing, showering or personal hygiene
  • Dressing or grooming
  • Toileting or incontinence care
  • Help with eating
  • Lifting, transfers or help standing and walking
  • Handling or giving medication
  • Nursing, wound care or any medical care

For hands-on care, pair us with a licensed home health agency or nurse registry. Many families use both. More on what we do and don't do.

How a referral works

  1. Send the referral

Use the form, or call us any time, even outside office hours. We need contact details and timing, not medical information.

  2. We call you

We confirm the timing with you, then reach out to the patient or family.

  3. We plan the first days home

We match a companion and set up the schedule with the family.
